There is a singular joy as a writer in the experience of having a talented artist bring a face to your character.  The authors of the Hiram Grange books were ecstatic to work with Malcolm McClinton, who was able to depict Grange's nuanced persona in a variety of subtle (and not so subtle) facial expressions.

Well according to The SCANDALOUS MISADVENTURES OF HIRAM GRANGE:

A confluence is, simply put, a crossroads of supernatural activity; an area of spiritual conflict within the greater War of Good Versus Evil. The Earth is spotted with thousands of minor (or roaming) confluences, but only twelve major (or fixed) confluences. 
A minor confluence can occur spontaneously whenever spiritual conflict breaks through to the corporeal plane. These usually manifest in unusual supernatural occurrences for a short period of time, including ghost sightings, poltergeist activity, or the appearance of physical omens. A major confluence, however, is firmly tied to a specific geographic area on a seemingly permanent basis.
